IMMIGRATION PHYSICALS (i-693)
USCIS-required medical exams for adjustment of status, completed by a designated Civil Surgeon
what is an i-693?
Form I-693, Report of Medical Examination and Vaccination Record, is a USCIS-required document for anyone applying for a green card (adjustment of status) or certain other immigration benefits. It must be completed and signed by a USCIS-designated Civil Surgeon — a physician specifically authorized by USCIS to perform this exam. The completed form confirms that an applicant meets the health-related requirements for immigration, including a review of vaccination history and screening for certain medical conditions.
What to Bring
Government-issued photo ID (passport, driver's license, or state ID)
Any immigration paperwork you have (receipt notice, prior I-693 if applicable)
Vaccination records, if available (this can shorten your visit and reduce cost)
List of current medications
Cost
$400
Includes both visits, required blood work and I-693 preparation
Does NOT include Antibody titers, Vaccines or Chest x/ray if needed
Vaccines
USCIS requires proof of certain vaccinations as part of the I-693.
We'll review your vaccination history at the start of your visit and let you know exactly what's needed.
If you don't have documented proof of vaccination, we can refer you to obtain missing vaccines , or you may opt to have a blood test to determine your immunity status (antibody titers).
Timeline
The Immigration physical process requires two separate visits. The initial visit (see below) and then the Physical exam with our Civil Surgeon.
Time between the initial visit and the Immigration physical can vary depending on vaccination status and testing results but can often be completed start to finish within 5-7 days.
Immigration physical exams are completed with sealed I-693 paperwork available the same day
Appointment Duration
Plan for approximately [30–60 minutes], depending on whether vaccines or additional screening are needed.
-
You will need to bring the following to your initial immigration physical visit:
Goverment-issued identification (passport, government ID, driver’s license) for all family members being seen, including children
All vaccine records (must be translated by an official medical translation service if not in English)
Lab testing/blood work will be completed during your initial visit
You will be sent a link to complete all necessary information for the I-693 from Medfiller. This information must be completed prior to your second visit.
-
Tetanus (current within the past 10 years - Examples: Tdap, Td, DTaP, DT)
Varicella (chicken pox)
Measles, Mumps, Rubella (MMR)
Polio
Hepatitis B
Flu vaccine (Oct-Apr)
If you do not have the required vaccinations, we will refer you to have these completed.
Blood tests to confirm immunity can be drawn at additional cost for Varicella, MMR and Hep B.
Some vaccines require multiple doses, but an initial vaccine is all that is required to complete the Immigration physical exam.
-
Tuberculosis Screening (Quantiferon blood test)
Syphilis blood test (Age dependent)
Gonorrhea urine sample (Age dependent)
Lab results take 3-5 business days to be resulted.
-
If the Tuberculosis screening test is positive a Chest x/ray is required to confirm that pulmonary tuberculosis is not present. We will order this for you to have completed at an imaging facility.
-
After ALL RESULTS AND VACCINES ARE COMPLETED, our staff will call you to schedule your second visit with the civil surgeon. Following the exam you will sign the I-693 and it will be sealed in an addressed envelope for you to submit to USCIS or your attorney.
For your convenience, 2 copies of the completed I-693 will be provided for you and your attorney as well.
