Mile High Medical Exams PLLC ("MHME," "we," "us," or "our")

Effective Date: 10/01/2026

Last Updated: 09/28/2026

Please read these Terms and Conditions ("Terms") carefully before using milehighmedexams.com (the "Site") or scheduling services with Mile High Medical Exams PLLC. By accessing the Site or booking an appointment, you agree to be bound by these Terms. If you do not agree, please do not use the Site or our services.

1. Nature of Services

MHME provides cash-pay outpatient medical examination services, including but not limited to:

●      DOT/CDL physical examinations (FMCSA certification)

●      USCIS immigration physical examinations (Form I-693)

●      Pre-employment and occupational physical examinations

These are examination and certification services, not ongoing primary care or emergency medical treatment. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.

2. No Doctor-Patient Relationship Through the Website

Use of this Site, including submitting a contact form or scheduling an appointment online, does not by itself create a doctor-patient relationship. A doctor-patient relationship is established only when you are seen in person for an examination. Content on this Site is provided for general informational purposes about our services and does not constitute medical advice.

3. Appointments, Scheduling, and Cancellations

●      Appointments may be scheduled online through our booking platform, by phone, or in person.

●      We request that you arrive on time with all required documentation (photo ID, relevant medical records, employer or immigration paperwork, and any items listed on our service pages).

●      Cancellations/rescheduling: (www.milehighmedexams.com/cancellation-policy) Please provide at least 24 hours' notice to cancel or reschedule. Late cancellations or no-shows may be subject to a fee of $50.

●      Late arrivals: Arriving significantly late for your appointment may require rescheduling at our discretion, to avoid delaying other patients.

●      Incomplete paperwork (e.g., an incomplete DOT medical history form) may result in delays to your exam or require a follow-up visit.

4. Payment Terms

●      MHME operates on a cash-pay basis only. We do not bill insurance or participate with insurance networks.

●      Payment is due in full at the time of service. We accept cash, credit/debit card, or HSA/FSA card.  Personal checks are NOT accepted.

●      Pricing for each exam type is listed on our Site and is subject to change; the price confirmed at the time of scheduling or check-in will apply to your visit.

●      Additional services requested during your visit (e.g., vaccinations required for an I-693, or follow-up testing) may incur additional charges, which will be communicated to you before they are performed where practical.

●      Refunds are handled as described below.

Refunds

●      Completed exams: Once an exam has been performed, no refund will be issued, regardless of the outcome (e.g., a DOT certification not issued, a request for additional USCIS documentation, or an employer's clearance decision). Payment is for the performance of the examination itself, not for a particular result.

●      MHME error: If an exam must be repeated due to an error on MHME's part (e.g., an incorrectly completed form), we will correct the error or repeat the applicable portion of the exam at no additional charge.

●      Billing errors: If you are charged in error or charged twice for the same service, contact us and we will correct the charge or issue a refund for the erroneous amount.

●      Cancelled appointments: Amounts related to cancelled or missed appointments are governed by our separate Cancellation and No-Show Policy, not by this refund section.

To request a billing correction, contact us using the information in Section 13 below.

5. Certification and Documentation Disclaimer

●      Completion of an exam does not guarantee a particular outcome (e.g., DOT medical certification, USCIS approval, or employer clearance). Certification and documentation decisions are made based on the applicable medical, regulatory, and legal standards (FMCSA, USCIS, or employer-specific criteria) at the time of your exam.

●      MHME is not responsible for delays, denials, or additional requirements imposed by USCIS, FMCSA, employers, or other third parties after your exam is completed.

●      It is your responsibility to bring complete and accurate information to your exam; incomplete or inaccurate information you provide may affect your certification outcome.
