  • A physical exam requiring typical vital signs, height and weight, and visual acuity. Your medical history will be reviewed including current medications, past or current conditions, and other injuries or limitations. Exam will include a general physical and final recommendations based on the exam findings. If you do not have a specific document, we will provide you with one that will be stamped and signed by a Medical Doctor.

    This exam does not include any blood work, urinalysis, pregnancy testing, EKG, or fitness assessment.

    COST - $80

  • The Basic physical exam as listed but without any blood work or EKG. This physical would include in-house urinalysis, pregnancy test or blood glucose, as well as any simple fitness or ability testing not requiring weights or props.

    COST - $125

  • A comprehensive Physical Exam that includes any required lab work (some limitations may apply), EKG, or extensive fitness testing.

    Blood work will be either sent to or drawn by an outpatient lab within our building. The results should be received within 3-5 business days, reviewed and added to your packet.

    Following this your paperwork can be completed, signed and either securely sent to you electronically or picked up in person if the hard copy with an original MD signature is required.
